     Primary amoebic meningoencephalitis (PAM), also referred to as Naegleriais is a hemorrhagic infection of the central nervous system with a 95% fatality rate (1,5). Naegleriasis is caused by Naegleria Fowleri, a thermophilic and amphizoic amoeba that can be found in soil and freshwater in a free-living state. It has been observed infecting immunocompetent young adults and children (as compared to granulomatous amebic encephalitis, an infection caused by another amoeba that infects primarily immunocompromised hosts) prior to nasal aspiration of warm water via recreational activities such as diving, swimming, and water sports (2,5). Infection occurs when the amoeba travel through the olfactory nerves and cribriform plate, where they then enter the brain and cause extensive hemorrhaging and necrosis. Incubation of N. Fowleri may last from 2 to 15 days, while death usually occurs 3 to 10 days from the onset of symptoms (3,6).

     Naegleriasis may be difficult to identify (often only being formally diagnosed post-mortem) due to the fact that the symptoms present similarly to viral or bacterial meningitis and progression of PAM is rapid. Identification is often done via microscopic examination of patients cerebrospinal fluid, while post-mortem a hematoxylin and eosin staining of brain tissue is preformed (4.3). Symptoms of a Naegleriasis infection include a stiff neck, fever, nausea, vomiting, seizures, coma, and death (4).

     There does not appear to be an accurate notation of the current number of cases worldwide, but Naegleriasis cases have been identified in less than 16 different countries. Also, there are few survivors of Naegleriasis, with about 5 current survivors in North America alone (4,6). An interesting case of survival comes from a 78-year-old male patient, an atypical age group for presentation of PAM (1). There is no set treatment for the disease, as there have been no clinical trials to assess which treatments are most effective. The main drugs to treat Naegleriasis are amphotericin B and miconazole, as they were seen to be most effective against the amoebae during in vitro testing (4,5). Though, amphotericin B has side effects such as renal toxicity on patients (2). Also, with recent research showing that miltefosine may be effective against N. Fowleri  as well; the CDC has made miltefosine available per an on-need basis for treatment of infections caused by free-living amoeba including Balamuthia mandrillaris, N. fowleri, and Acanthamoeba species (2).
