Passing this on.

I am a Family Doctor and I want to keep a promise made to a patient.

Julie was a healthy, post-menopausal woman in my care who came in for a periodic health examination. One of my routine questions, in what is called the “Review of Systems”, was to ask if she had experienced any vaginal bleeding.

She said “No” but then laughed and added, “Other than when my period came back for a few months last year”.

All health care professionals are taught early on that ‘vaginal bleeding in a post-menopausal woman is Cancer of the Uterus until proven otherwise’. This comment by Julie was, therefore, a red flag (no pun intended) prompting further questions, an examination and an ultrasound of her pelvis.

Julie was surprised to see me so concerned, especially since the symptoms had not recurred over many months.

Sure enough, a pelvic ultrasound and tissue sampling confirmed Cancer of the Uterus.

Julie underwent a hysterectomy and radiation therapy. She is now healthy, cancer-free and is expected to stay that way.

After all this was done, Julie sat ME down for a talk. She told me she’d had no idea a ‘short return’ of her period after menopause was a danger signal. Furthermore, she addressed the topic with friends over coffee and discovered that, out of 20 women, NONE of them knew this symptom was abnormal! She admonished me to “Tell women this! Don’t assume we know it!”

From that day on, I have kept Julie’s advice in mind when talking with post-menopausal patients. But recently my wife suggested that I should take this to a wider audience.

So, Julie, this is for you:

If you are a post-menopausal woman and your period ‘comes back’ or you have even one episode of vaginal bleeding, TELL A HEALTH CARE PROFESSIONAL and insist on having it investigated!

Wishing you all good health and long lives.

This is seriously gorgeous! So talented.

"This is your fault." Silver whined. He lay on his back staring up at the ceiling of the cell. He'd start whining about the filth on his suit when he sat up again. Vale leant his forehead on the bars and slowly exhaled. "You just have to poke your nose in, don't you."

"And you have to break the law, don't you?" Vale sighed. "Should I just give up and pretend that you don't exist?" Silver shot up, sitting bolt up right.

"Why on earth would... What have I been laying in?"

"Four years of dust and dirt. I hypothesise that these rooms flood when the river gets too high, like it did after the storm we had two weeks ago. Hence why the floor is still damp and the plaster is damaged. Four years of dust and dirt, mildew and whatever the river dragged in."

"Ugh. You could have stopped with dust." He climbed to his feet and joined Vale at the bars. "What are you looking at?"

Vale turned to him. "A nuisance." Silver blinked slowly, like a cat.

"How rude."

"I apologise if it hurts to hear the truth." Vale replied, looking away from him. "I'm trying to work out how to get us out of here since you're more worried about your clothes." Silver ran his hand over the shoulder of his suit and wrinkled his nose in disdain at the filth that clung to his fingers. He stared at his hand, and then at Vale's shoulder, and then back down to his hand. He smeared the dirt off and onto Vale's sleeve. Vale jumped back.

"What on earth are you doing?"

"My hand was dirty."

"Your face will be dirty if you keep this up." Vale hissed. "Will you shut up so that I can get us out of here? Or do you want to remain in this cell forever?"

"And spend my days with you? Yes please." Vale muttered something under his breath that seemed to rhyme with 'Silver you piece of quit.' not that Vale would often use such profanity when there were people to hear him, but sometimes the situation, or person, called for it. As long as Winters couldn't hear to call him a hypocrite.

"Have you tried the door handle?" Silver suggested, before trying it himself.

"You're a genius." Vale snapped when the handle wouldn't even budge. "What's your next solution? Turning semi corporeal and slipping through the bars?"

"How stupid do you think I am?" Silver asked, before. "Nevermind. Don't answer that." Vale rolled his eyes.

"Now, unless you have any bright ideas, please be quiet."

"Shout for help."

"Pardon?"

"Shout for help. Tell them I'm not breathing or something and the guard will come to investigate and open the door."

"Why would that work?"

"I saw it in a movie."

"A what?" He sighed.

"Get Winters to show you." Silver said. "Do you want to try that then?"

"I suppose that it wouldn't hurt. You'll have to lay on the floor again."

"I'm already dirty, a little more won't kill me. Hopefully." Silver said, he sat down where he had been before, before carefully laying down with his back to the cell bars. Vale took a deep breath.

"Guard! Guard! He's not breathing!" He waited and a guard started to slowly stroll closer. "He's going to die. What have you done to him?" The guard peered through the bars.

"Will you be quieter if he's dead?" The guard asked. Vale glowered at him. "Our instructions were to keep you here. Nothing about keeping you ali-" Vale's hand shot through the bars and grabbed a fistful of his hair, using it to slam the man's head against the metal bars until he went limp and Vale released him to fall to the ground.

"That works too, I suppose." Silver said, getting up, he went to brush the dirt off himself before remembering. Vale rifled through the guards pockets, finding a small flick knife, a pistol and the keys. The cell key was the sixth that he tried.

"Let's get out of here."

Vale more than anyone knows he’s not a sensible person in charge.

Li Ming: where is everybody?

Vale: Irene had a nervous collapse, Kai is looking after her, Silver went back to bed, and I’m in charge.

Li Ming: fuck.

Vale: i know right?

This is fabulous! You’re is hilarious and I love their dynamic, especially Vale leaping from the chair like his life depends on it. Thanks so much for writing.

Hiya, could you do number 1 on the quarantine writing prompts for Silver and Vale please?

“You. Me. Quarantined for two weeks. Anything could happen.”Silver purred, leaning on the arm of Vale’s chosen armchair, well stuffed, near to the window, within arms reach of a coffee table. It was dark red.

“Yeah. I may kill you. Or we may both get sick.” Vale dryly said, he was flipping through a notebook, going over the notes of a recent case that would hopefully be going to trial when the quarantine was over. “Can I help you? You are in my light.” Silver got up, walked around the back of  the chair, dragging his fingers across Vale’s shoulders and back, Vale suppressed a shiver. He sat on the other arm.

“Is this better?”

“If you insist.” He said dryly. “Are you just here as an irritating distraction? Or do you actually have some purpose in being here?”

“This is my home.”

“Yes. You are an incredibly observant man. So you have any other shockingly new observations? Anything else that I may have missed?”

“I like this armchair.”

“I can tell. There were three strands of your hair on it when I sat down, and your slippers in front of it. I assume that the position by the window was intended for you to be able to watch the streets.” Vale said, flipping his page. “You like to know what is going on at all times and the view allows you to see who is coming to your door, and when they leave as well.”

“Anything else?”

“It was recently reupholstered, suggesting that you frequently throw yourself down onto it in a pique of melodramatic fitting.”

“Actually it was because I happen to enjoy bringing my bed partners up to this room.” Vale jumped out of the chair, stumbling over Silver’s slippers. “Oh, good lord you are gullible.” He slipped off the arm and landed over the chair. “No, you were… right the first time.” Vale had dropped his notebook in his rush to get out of the seat and Silver picked it up and held it out to him. “Thank you for the entertainment though.”

Vale snatched the notebook out of his hand. “You disgust me.”

“Would whiskey make you feel better? There’s a drinks cabinet over there.” He waved his hand toward the sideboard at the side of the doors. “If you fancy one, feel free to pour me a glass too.” Vale was going to snap back at him that he wasn't a servant, but a glass of whiskey sounded like a good idea, so he opened up the cabinet and plucked out the first bottle that he found, holding it up to examine it.

“This is good stuff.” Vale mainly said to himself as he unstoppered a bottle and splashed some into a glass.

“Of course it is.” Silver replied with an idle flutter of his fingers. “I have superb taste in alcohol, amongst other…” He watched Vale bend over to put the bottle away again. “Things.” He finished.

“I doubt you have good taste in most things.” Vale replied. He put the glass within Silver’s reach and kept the bottle in his hand. “I will give you this though. Thanks for the drink.”

“You’re welcome!” Silver called as Vale let the door shut with a thunk behind him.
